Pneumoperitoneum in Acute Diffuse Perionitis due to perforation of the Gastro - intestinal Tract



Abstract
A study was made on the pnemoperitoneum in acute diffuse peritonitis due to perforation of the
G-I tract treated surgically at the Chinhae Naval Hospitak from January 1963 to July 1969.
Among 104 cases, 61 cases were peptic ulcer perforations 21 cases were perforations of appendix
due to appendicities, 13 cases were perforations due to typhoid fever and the remainders were
perforations of the jeejunum and the colon due to blunt trauma.
Upright chest film and abdominal films(plain abdomen and left lateral decubitus) were made and
compared in each case.
1) Pneumoperitoneum was seen with the highest incidence in the peroration of the duodenal
ulcers(78.0%).
2) Pneumoperitoneum in ileal perforations due to typhoid fever was 76.9% and in the perforation
of the stomach ulcer was 65.0%.
3) Perforation of the jejunum by blunt trauma presented pneumoperitoneum in 28.6%.
4) Small pin-head sized perforation of the colon presented no pneumoperitoneum.
5) pneumoperitoneum was seen in a case of appendicitis with perforation.
6) The overall incidence of the pneumoperitoneum was 56.7%.
7) The cases showing pneumoperitoneum presennted it 98.3% in the upright chest film and among
these 56.6% presented pneumoperitoneum also in the abdominal films.
8) 98.3% of the pneumoperitoneum was seen in the right subphrenic space.
9) The best rentgenological view to demonstrate the pneumoperitoneum was upright chest P¡¤A
view taken after being kept in erect position for about 5 minutes or longer.
